Job Overview We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The Star+Plus (LTSS) Service Coordinator Level 1 is responsible for overall management of member's case within the scope of licensure; provides supervision and direction to non-RN clinicians participating in the member's case in accordance with applicable state law and contract; develops, monitors, evaluates, and revises the member's care plan to meet the member's needs, with the goal of optimizing member health care across the care continuum. StarPlus Service Coordinator Level 1 Licensed RN Location: 77081- Harris County (Local Only) can apply Must reside in Harris County, Texas Must be willing to travel in the service area NO RTO in the 1st month of training Local Travel rates 40hours per week (within 30 miles from the facility) $47.50/hour $71.25/hour - Overtime Rate $1,900 Weekly Gross For RN position - candidate needs to be within 30 miles of the five cities below and must provide a Driver’s License in the submittal profile to show proof of residency in these areas: o Clear Lake o Galveston o Seabrook o Baytown o Channelview · Required to go into the office every day from 8am-5pm CT for the 1st month for training (location is 4888 Loop Central Dr. Suite 600 Houston, TX 77081) · Must have Managed Care Organization experience · Bachelor’s or Master’s degree based on required experience in health, psychology, sociology, social work, nursing, or another relevant human services field. · 3-5 years nursing experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role Job Types: Full-time, Contract, Temporary Pay: $1,900.00 per week
